What do I need to bring to my appointment?

You need to bring your insurance card, driver’s license, any x-rays, cat scans and MRI’s that have been done.

Can someone else bring my child to the appointment?

Yes, if you have put them on your HIPAA form stating they can seek care for your child. If it is your first visit then you will need to send a note stating that this person can seek care for your child, with a copy of your driver’s license and insurance card.

What happens if I can't attend my scheduled appointment?

We ask that you give our office at least 24 hour notice of a cancelled appointment. As of January 1, 2021, there is a $25 no show fee if visits are missed without 24 hour notification. Our office can be reached at 817-865-6950.
