What do I need to bring to my appointment?

You need to bring your insurance card, driver’s license, any x-rays, cat scans and MRI’s that have been done.

Can someone else bring my child to the appointment?

Yes, if you have put them on your HIPAA form stating they can seek care for your child. If it is your first visit then you will need to send a note stating that this person can seek care for your child, with a copy of your driver’s license and insurance card.

What happens if I can't attend my scheduled appointment?

We ask that you give our office at least 24 hour notice of a cancelled appointment. As of August 30, 2022, there is a $50 no show fee if visits are missed without 24 business hour notification. Our office can be reached at 817-865-6950.

Appointment Notice

Please note that when teenagers visit the practice someone over the age of 18 must be present.
